Fort Collins Symphony Attendance Policy
The Fort Collins Symphony requires that all patrons provide proof of full vaccination* against COVID-19 with photo/student ID OR a negative COVID lab** test taken within 72 hours of the performance start time with photo/student ID.
PROOF OF VACCINATION
Acceptable proof of vaccination may include one of the following that matches a government issued photo I.D. or school I.D. for youth
A physical copy of a WHO (World Health Organization) or FDA-approved COVID-19 Vaccination Record Card
A high-resolution photograph of the front and back of your WHO/FDA issued COVID-19 Vaccination Record Card on smart phone
Proof of your vaccination card on the MyColorado mobile device app
*People are considered fully vaccinated for COVID-19 14 days or more after they have received the second dose in a 2-dose vaccine series currently authorized for use by the FDA and WHO OR 14 days or more after they have received a single-dose vaccine that is currently authorized for use by the FDA and WHO.
PROOF OF NEGATIVE COVID LAB TEST
Acceptable proof of a negative COVID-19 viral test result may include PCR and antigen tests, including rapid tests, from recognized test providers or laboratories that show the patient’s name, type of test performed, date of test and negative test result along with a government issued photo ID or school ID for youth. Acceptable forms include:
A printed document from the test provider or laboratory showing the negative test result
An electronic test result (email, text message, or mobile application record) displayed on attendee’s phone or mobile device.
**The Fort Collins Symphony does not accept home tests as proof of a negative COVID result.
MASK REQUIREMENT FOR ALL PERSONS
Per the public health order as issued by Larimer County, all persons three years of age and older must wear masks. This includes patrons, volunteers, staff and vendors. Patrons may unmask when actively consuming food or beverages where permitted in the venue.
Patrons who are unable to adhere to these policies should contact the Box Office to discuss their options.
If you are unwell, please stay home and contact the Box Office prior to your performance to discuss ticket options. If you, or any members of your party, are experiencing symptoms similar to those of COVID-19 or if you have had known exposure to someone with COVID-19, please stay home.

Utilizing MyColorado, the state of Colorado’s official mobile app, you can digitally access your vaccine record from your cell phone. Click here and follow the instructions. It's easy, fast and super convenient.
Any patron needing an accommodation due to a medical condition, disability, or a sincerely held religious belief, practice, or observance that prevents them from taking the vaccine will be admitted with the following:
Proof of a negative COVID-19 PCR test taken within 72 hours of the performance start time, or a negative COVID-19 antigen test taken within six hours of the performance start time that was administered by an official testing center. Home testing kit results will not be accepted for entry.
Masks are required regardless of accommodation for all patrons ages 3 and older.
